Photo Release

The undersigned hereby authorizes Golden Heart School of Nursing, or its designee(s) while attending at Golden Heart School of Nursing and/or as a participant in a Golden Heart School of Nursing Program. The undersigned agrees that Golden Heart School of Nursing or its designee may use or permit other persons to use the video and/or visual recordings, negative or prints prepared for such purpose and in such manner as may be deemed necessary, including but not limited to promotional and publicity materials and Golden Heart School of Nursing internet websites, Google, Facebook business account, and other social media platform.

The undersigned hereby waives any right to inspect and/or approve the finished product or products made from such recordings and photographs and waives any claim to royalties or any monetary compensation for appearing in such recordings, photographs, and publications.  

This consent form shall be binding on all heirs, successors and assigns and represents the entire agreement between the undersigned and Golden Heart School of Nursing.
