e-Learning Enrollment Agreement

This Enrollment Agreement is entered into between Golden Heart School of Nursing (“School”) and the undersigned Student enrolling in the eLearning Nurse Assistant Training Program (6 weeks).

Section 1. Program Overview

The Program is designed to provide the knowledge and skills required to become a Certified Nurse Assistant. The student agrees to the following:

1. Submit and comply with all required documentation and prerequisites online through the website.
2. Fulfill all required hours, coursework, and evaluations.
3. Maintain grading requirements as stated in the student handbook.

Section 2. Student Responsibilities

The student further agrees to:

1. Abide by all School policies and procedures (Student Handbook).
2. Maintain professionalism in conduct and communication at all times.
3. Comply with all applicable confidentiality laws including HIPAA by protecting patient information and no unauthorized disclosure.
4. Demonstrate an understanding of abuse and elderly abuse, including recognizing, reporting, and responding appropriately to such situations.

Section 3. Consequences of Non-Compliance

Failure to follow this enrollment agreement — including attendance, academic performance, or conduct standards — may result in dismissal or non-completion of the Program.
