Credit Card Information Authorization, Use, and Retention Policy

Section 1. Policy Statement

Golden Heart School of Nursing maintains this policy to ensure that the collection, authorization, use, and retention of student credit card information is conducted in a manner that is transparent, voluntary, secure, and consistent with applicable consumer protection standards.

Section 2. Permitted Use of Credit Card Information

Upon acceptance by the student, Golden Heart School of Nursing will use stored credit card information solely for the purpose of processing tuition fees. No charges outside of those disclosed in the student’s enrollment agreement, payment plan, or written authorization shall be processed.

Section 3. Storage and Data Security

Golden Heart School of Nursing utilizes secure, PCI-compliant third-party payment processors for all electronic payment transactions. The following safeguards are maintained:

1. Full credit card numbers, CVV codes, and sensitive authentication data are not stored by the school in plain text.
2. Access to payment information is limited to authorized administrative personnel with billing responsibilities.
3. Credit card information is retained only for the duration of the program until all financial obligations are settled.

Credit Card Authorization and Consent Form

Golden Heart School of Nursing

I authorize Golden Heart School of Nursing to retain my credit/debit card information through a secure, PCI-compliant payment processor for the limited purpose of processing tuition fee: 

Registration fee of $100.00, 

First Payment of $400.00 to be paid on or before the 1st day of the online theory class.

Second Payment of $500.00 be paid on or before the 7th day of the online theory class.

Third Payment of $500.00 be paid on or before the 1st day of the clinical training.

Fourth Payment of $500.00 be paid on or before the 7th day of the clinical training.

I Acknowledge and Understand That:

1. I am responsible for maintaining valid and current payment information and sufficiency of funds.
2. The student is responsible to fulfill payment as scheduled if the credit card is declined.

By accepting the above terms and conditions, I certify that I have read and fully understand this authorization and consent to its terms.
